Overview

Chondroitin sulfate, commonly referred to as chondroitin, is a naturally occurring glycosaminoglycan found in cartilage. It is often extracted from animal sources like bovine, porcine, or shark cartilage and is primarily used as a dietary supplement to manage symptoms of osteoarthritis, particularly knee osteoarthritis (KOA). Chondroitin is believed to support cartilage resilience and inhibit its degradation. Research on chondroitin is moderately to highly mature, with numerous randomized controlled trials (RCTs) and systematic reviews available. While some meta-analyses indicate modest benefits in pain relief and improved joint function, the quality and consistency of evidence vary. It is classified as a symptomatic slow-acting drug for osteoarthritis (SYSADOA).

Benefits

Chondroitin sulfate has demonstrated statistically significant pain relief in knee osteoarthritis compared to placebo, with meta-analyses showing an effect size of approximately -0.54 cm on pain scales (95% CI, -0.90 to -0.18 cm). It also improves physical function in KOA patients more effectively than placebo. How it works

Chondroitin sulfate is thought to contribute to cartilage matrix integrity by providing building blocks for glycosaminoglycans and proteoglycans, which maintain cartilage elasticity and hydration. It may inhibit enzymes that degrade cartilage and reduce inflammation in joint tissues. The primary biological pathways involve interaction with matrix metalloproteinases (MMPs) and inflammatory mediators. Oral bioavailability is moderate; chondroitin sulfate is absorbed and distributed to joint tissues but with variable efficiency.

Side effects

Chondroitin sulfate is generally well-tolerated with a good safety profile. Common side effects (occurring in >5% of users) include mild gastrointestinal symptoms such as nausea or diarrhea. Uncommon side effects (1-5%) include rare allergic reactions. No serious adverse events are consistently reported (<1%). Caution is advised when using chondroitin with anticoagulants due to potential mild blood-thinning effects. Contraindications include known hypersensitivity to chondroitin or its animal sources. Dosage

It is recommended to take chondroitin daily with meals to improve tolerance. It is available in oral capsules or tablets. Co-administration with food may enhance absorption.
